JotForm claims to be the quickest and easiest way to build an online form. This includes unlimited form creation, unlimited fields per form and reports, up to 10 MBs of storage space, up to 100 monthly form submissions, up to 10 SSL secure submissions per month and up to 10 payments received per month. The mid range Premium membership costs around $10 per month or around $90 per year. A bi yearly package is also available for just under $140 for two years. This package includes 1 GB of storage space, and upgrades all monthly submissions to 1,000. The Professional membership costs around $50 per month or $500 per year. The bi yearly option for the Professional costs around $800 for two years. This plan upgrades to include up to 100 GBs of storage space and increases all monthly submissions to 1,000,000.
